| On May 16, 2022, Licensing received a complaint alleging concerns with care and supervision. The complainant provided the following details: - The family was not made aware of the multiple falls the PIC sustained. All areas of the complaint were thoroughly investigated. Evidence was received and reviewed from interviews, correspondence with the facility, and inspection.
