-Follow-up inspection to Public Health Act Correction Order issued August 2, 2019. -New operator has contact the Health Department by telephone. New operator stated no physical renovations are being carried out to the facility. New operator is cleaning up facility and organizing the store. -Current owner will no longer be part of the business -In order to change ownership the following information is required: Health application form, copy of Certificate of Incorporation, copy of valid FoodSafe Level 1 certificates, food safety plan, sanitation plan, floor plan (sketch of layout), and health operating permit fee (based on when facility re-opens). -Conditions of the Public Health Act Correction Order issued August 2, 2019 have been met. Correction Order has been rescinded. -Please contact the inspector if you have any questions or concerns.
