201 - Food contaminated or unfit for human consumption [s. 13] Observation (CORRECTED DURING INSPECTION): Four take out containers with moldy food stored in the walk-in cooler. Mice droppings observed on vegetables and in a bowl of raw chicken. Photographs taken at this time. All foods discarded at the time of inspection. Corrective Action(s): The operator must take care of the walk-in cooler (regularly check food - first in first out/label with dates, as well as clean storage shelves and cavities of the cooler). All contaminated/spoiled food must be discarded immediately when observed. Violation Score: 25 302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)] Observation: A bottle of empty household bleach was connected to the low temperature dishwasher. No dishwasher sanitizer was available on site. Photographs taken at this time. Corrective Action(s): Service the dishwasher. Chlorine sanitizer must be at 50 ppm after final rinse. Commercial chlorine sanitizer must be connected. Violation Score: 25 402 - Employee does not wash hands properly or at adequate frequency [s. 21(3)] Observation: Liquid soap and paper towels are unavailable in the staff washroom. Corrective Action(s): The liquid soap and paper towel must be available in the staff washroom all the time. Violation Score: 25

304 - Premises not free of pests [s. 26(a)] Observation: The premise severely infested with rodents. Droppings were observed throughout the facility. Observed a mouse in-glue-trap between wall and side of the walk-in cooler. Photographs taken at this time. Corrective Action(s): A professional pest control company must be contacted for a full inspection. Violation Score: 15 305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)] Observation: Holes, cracks ,and missing or not properly attached floor/wall junctions observed throughout the facility. Photographs taken at this time. Corrective Action(s): Repair/replace floor/wall junctions. Seal all holes and cracks as discussed at this time. Violation Score: 15 306 - Food premises not maintained in a sanitary condition [s. 17(1)] Observation: Grease build up and food debris observed on surface between cooking equipment and hard to reach areas. A Closure Order issued at the time of inspection. Premise is required to be closed immediately. General sanitation found to be poor at the time of inspection. Unpleasant odour noticed from the back area and in walk-in cooler. Food debris, grease, and rodent (mice) droppings found throughout the food premise as well as in the walk-in cooler and staff washroom. Note: Food debris provides a food source for rodent. The entire kitchen requires cleaning to remove food debris and droppings. (Ensure droppings are sprayed with bleach (1cupful/gallon of water) prior to cleaning. Do not sweep or vacuum droppings.) Kitchen handwashing station supplied with hot/cold running water, soap, and paper towels. Coolers (2 upright coolers, walk in) at < or = to 4 C. Hot holding units (rice and soup) > 60 C. Freezers at - 18.0 C. A Closure Order was posted on the front door. Photograph taken. Do not remove or cover this Order as discussed at this time.
