Observation (CORRECTED DURING INSPECTION): Observed individually portioned mayo for take out and a squeeze bottle of mayo stored in a temperature danger zone. The operator relocated mayo to a cooler.

Observation (CORRECTED DURING INSPECTION): Observed two small tongs stored directly on the bottom of a sliding door cooler. Tongs removed at this time.

Observation (CORRECTED DURING INSPECTION): A plastic container labelled "milk powder" stored away without being air dried. Container removed to be washed and sanitized again. Ensure food contact surfaces are air dried completely prior to storing.

Observation (CORRECTED DURING INSPECTION): Hot water was not available at the hand washing station next to a ramen station. The operator serviced the faucet at this time. The hand washing sink now equipped with hot and cold running water, liquid soap and paper towels.

Observation: 1. Raw pork cubes stored on top of other ready-to-eat foods in a glass sliding door cooler. Raw pork relocated at this time.
2. Food stored in its original can after opening.
3. Condensation from a pipe in a walk in cooler. Processed foods stored underneath.
4. Stainless steel bowls without a handle is being used as a food scoop. The scoop removed at this time.
Corrective Action(s): 2. Relocate foods in a food grade container.
3. Remove foods stored under the condensed pipe.

Correct this by: immediately

Observation (CORRECTED DURING INSPECTION): Rodent droppings noted in the following areas:
- Dry storage room: under the shelves, corners and hard to reach area and inside one of the plastic crate.
- Under the tool storage shleve.
- Behind hot water tank.
- Next to a standalone freezer near the hot water tank.
- Bar: under counter cabinet where boxes of beer are stored.
- Kitchen: behind a four door freezer, under the dishwasher, bottom shelf located in the kitchen back hallway.

Operator initiated cleaning procedure at this time. All droppings cleaend at this time.

Observation (CORRECTED DURING INSPECTION): The following food contact surfaces are discarded at this time:
- 5 plastic containers with holes blocked by duct tapes.
- Damaged stainless steel bowl with duct tape.

Freezers at adequate temperature.
Hot holding units measured 60C or higher.
Food contact surface sanitizer measured at 100 ppm chlorine.
Low temperature dishwasher's sanitizing cycle was measured at 100 ppm chlorine at the plate level.
