Corrective Action(s): Utensils were relocated at the time of inspection. Do not keep kitchen utensils under soap dispenser as it can get contaminated during hand washing process.

Corrective Action(s): Do not serve Soup until Food Safety Plan has been submitted and approved. Operator communicated verbally via telephone that soups will no longer be served at the current premises.
