Observation (CORRECTED DURING INSPECTION): Chopsticks and ladles observed stored in stagnant room temperature water.
Corrective Action(s): Do not store utensils in luke warm water left out at room temperature. Use clean utensils that have been washed in the dishwasher or store frequently used utensils in an ice bath at or below 4 degrees C. Instructed owner to wash chopsticks, ladles, and scoosp in the high temperature dishwasher then stored in an ice bath.

Observation (CORRECTED DURING INSPECTION): Newspaper used to cover vegetables in the walk in cooler. Moistened cloth used to cover chopped vegetables.
Corrective Action(s): Do not use newspaper or cloth to cover foods. Ensure food is protected from contamination at all times.

304 - Premises not free of pests [s. 26(a)]
Observation: One mouse observed caught in mechanical trap that was placed underneath the shelf to the left of the high temperature dishwasher. Some droppings observed in the corner by the deep fryers. One caught in trap left in hallway. Reviewed recent monthly pest control reports. Pest control technicians report from February 7, 2018 stated one mouse caught.
Corrective Action(s): Keep the back door leading to the parking lot closed at all times. Continue with regular cleaning underneath and behind all equipment on a frequent basis. Continue keeping the additional screen door to the main door leading into the premise from the hallway. Followup inspection March 29, 2018.

Observation: Food debris accmulations observed inside of several freezers.
Corrective Action(s): Thoroughly clean and sanitize the interior of the freezers. Focus cleaning efforts on the lowest shelf and the gaskets to the doors. Correct by March 8, 2018.

Hand sinks for kitchen, sushi bar, and serving areas provided with liquid soap and paper towels.
100ppm bleach available for use in the sushi bar and main kitchen. Reminder to store all wiping cloths in this solution when not in use.
Dishwasher registers 75 degrees C at the plate surface during the final rinse cycle.
Improvement in general sanitation. Continue regular cleaning on a regular basis. Focus cleaning efforts underneath and behind all cooking equipment and shelves.
