Staffs cleaned the ice machine but many pieces were still observed to have black debris inside.
Directed owner to request for professional service to clean the ice machine, post sign outside of machine to use bagged ice (staff purchased immediately) until issue is resolved.
Ice machine door also observed to have mildew and needs to be cleaned.
Aprons were being used or hung at time of inspection.
Raw chicken not being prepared at time of inspection.
Kitchen handsink is not blocked.
New seal on pipe added and kitchen handsink no longer leaks.
Food was covered in the coolers.
Dishwasher was replaced with a new unit and was tested to reach 78.9C at plate, see thermo-label.
In the future, please refrain from making structural or equipment change without approval.
Please send the dishwasher spec sheet by e-mail to undersigned to verify if the unit is approved. No logos on certification was able to be identified at time of inspection.
Thermometer used by staff also appears to be malfunctioning as it is difficult to read the numbers. Please replace with a NSF approved, waterproof thermometer with max/min mode.
Small fridge under fryer has been cleaned but mentioned to operator that this back area needs more attention as there is a lot of grease accumulated.
Sushi bar floor maintained.
Additional metal board added to top of back door to deter pest entry.
New carpet added to office but explained to owner to be cautious of rodent entry as door gap underneath is large enough for them to enter. Keep food out of the office and door closed after use.

->To conduct follow up inspection in 3 weeks, please have ice machine properly cleaned and general sanitation maintained.
