2. Ice scoop stored in cardboard box in walk-in cooler.
Corrective Action(s): Ensure that food is stored in a sanitary manner. Tray removed and placed on rolling cart during inspection.

Corrective Action(s): Ensure that premises is cleaned on a regular basis and as needed.
Clean up droppings in food premises.
Clean/remove cobwebs along storage.
Clean out freezer.
