Observation (CORRECTED DURING INSPECTION): Some fries in walk in cooler was not covered; ice in freezer was not covered. Staff was touching personal cell phone when raw chicken was being handled
Corrective Action(s): Ensure all food is properly covered to prevent contamination
Ensure to store personal items away from food prep area to prevent contamination of food from those items.

Low temperature dishwasher had residual chlorine concentration at 50 ppm on plate level
Sanitizer Quaternary Ammonium was available at 200 ppm
Glassware dishwasher had residual iodine concentration at 25 ppm
Cold holding units were at 4 C / 40 F or colder
Hot holding units were at 60 C / 140 F or hotter
Freezer units were at -18 c / 0 F or colder
Foodsafe level 1 trained staff was onsite a the time of inspection

Note: operator to keep the lid of prep cooler closed when not in use to maintain temperature of food below 4 C
