Follow up inspection conducted.
Dishwasher technician was onsite and had repaired the unit. Detergent and chlorine sanitizer available. A chlorine concentration of 100ppm was achieved a plate level. Operator is to rewash all dishware, utensils, and equipment through the dishwasher.
Operator is to obtain chlorine test paper to monitor dishwasher daily to ensure it is in good working order.
Written pest report for the treatment of cockroaches were present for the following dates: Aug 24, 2017, Nov 1, 2017, Nov 20, 2017, & Dec 1, 2017. Operator had also scheduled for fumigation of the premises on March 30, 2018 during the closure of the business. Once the fumigation is complete, ensure all food contact surfaces are properly washed and sanitized.
Written report from pest company is to be presented during the follow up inspection on Apr 3, 2018.
Operator is to continue to submit pest reports to the health department.
