Met with owner and he submitted the following: application form copy of the legal documents need to be emailed to me permit fee was paid under the new owners name (by Lisa) 2 food handlers have F.S. Lev. 1 training. Please ensure that at least one of the person in the kitchen is trained. Menu is same, no additional food items. Sanitation plan to be submitted by July 19, 2018. F.S. plan for review. Daily temperature logs are available. New owner is not proposing any changes to the floor plan. Please just ensure to ask for approval before you do any changes on the approved floor plan. Note: Translate documentation must be available onsite.
