Observation (CORRECTED DURING INSPECTION): No sanitizer available onsite. No bleach.
Corrective Action(s): Operator purchased bleach at time of inspection. Demonstrated proper mixing ratio to operator -> 1/2 tsp bleach to 1 litre of water to make 100ppm sanitizer. This must be used to sanitize food contact surfaces.

Observation: Premises added menu items (potentially hazardous food - refer to menu items listed on the report) and started using glassware without Health Approval.
Corrective Action(s): Discussed with owner and they have decided to revert back to the original menu - prepackaged tea served in paper cups only. Please e-mail me to confirm this change. Cheesecake, hummus, guacamole and ice cream discarded onsite. Glassware to be removed from premises. If you plan to upgrade your menu to include potentially hazardous food, you will need to upgrade requirements to obtain an Operating Permit and a commercial dishwasher.

Comments

Owner has added menu items: cheesecake, baklava, banana bread, mixed nuts, hummus and guacamole salsa + chips (all from supplier, not made onsite).
Prepackaged juice, pop and tea available (tea bags). Photo of menu taken at time of inspection.
Found cheesecake, hummus and guacamole in fridge only at 4C (main fridge). Thermometer present.

Handsink: soap, paper towel, hot & cold running water available
Small cooler next to the hallway handsink is for staff use only - please mark.
Two compartment sinks operational with soap and paper towel
Commercial dishwasher is still absent
No apparent signs of pest noted at time of inspection
Herbal hookah only, disposable mouthpieces available
No cut fruits for hookah. No tobacco sold.

Summary of requirements:
1. Remove glassware from premises.
2. Update menu and submit a copy to me.
3. Provide written confirmation (e-mail) that you will remain as hookah lounge without providing potentially hazardous food and will refrain from using glassware.
4. Clean the kitchen floors and counters.
