304 - Premises not free of pests [s. 26(a)] Observation: Pest control technician was on-site sealing potential entry points at time of visit. Pest control technician has more areas to work on. Fresh and old mouse droppings are noted - in storage room near dining area; by dishwashing area; by main cooking line; on floor under shelving racks; and near staff lockers. Corrective Action(s): Remove all droppings, spray affected areas with bleach sanitizer, and mop all affected areas. Operator should discuss pest problem with building management. Violation Score: 15

Follow-up inspection to routine inspection conducted and closure issued on September 02, 2022. Closure order remains. In order to re-open: 1. Increase pest control visits to twice a month, until pest problem is under control. New agreement must be provided at follow-up inspection. 2. A thorough inspection by a pest control technician with report. A report must be provided at follow-up inspection. 3. Identify new holes or entry points and seal all. 4. All droppings must be removed. *No signature is required at this time.
