Observation: Front gate door sweep has been chewed through allowing for pest entry.
Corrective Action(s): Replace front gate door sweep. It is recommended door sweep be replaced with pest proof model (Xcluder) available through pest control technician.

Observation: Ice machine - visible mold growth on interior

Fruit fly activity noted around 3 compartment sink area on walls and under sinks.

Mouse droppings observed - under dry storage shelving for take-out containers, behind pillar next to walk-in cooler, by grease trap adjacent to back hand sink, under front 2 door undercounter cooler, and around front pillar by swing gate entrance.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.

Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine 1 tsp bleach / 1 liter water) after cleaning.
