Observation: 1. Accumulation of mildew / biofilm observed on interior surfaces of ice machine, on the panel cover located near the top of the ice machine. Note: Ice was not observed to be directly in contact with biofilm / mildew.
2. Few instances of equipment / utensils (e.g. one glass, vegetable chopper, utensil bin) observed with small quantity of food debris.
Corrective Action(s): 1. Discard ice. Ensure ice machine interior is thoroughly cleaned and then sanitized. Allow surfaces to air dry before use. Date to be corrected by: Today.
2. Equipment / utensils washed and sanitized during inspection, or set aside to soak before being washed and sanitized. Ensure all equipment and utensils are properly washed, rinsed, and sanitized before storing away.

Observation (CORRECTED DURING INSPECTION): Sink and Surface Sanitizer was measured from the dispenser located in the dish pit at approximately 848 ppm DDBSA / 1875 ppm lactic acid, which is slightly above the required concentration range of 272 to 700 ppm DDBSA / 704 to 1875 ppm lactic acid.
Corrective Action(s): Sanitizer buckets and spray bottles containing Sink and Surface sanitizer above the required concentration range to be emptied and refilled at the sanitizer dispenser located near walk-in cooler (dispenser set to correct concentration).

Observation: 1. Small visible gap observed under back door.
2. Accumulation of water on floor near ice machine and in dish pit.
Corrective Action(s): 1. Ensure above-noted gap is properly sealed to prevent pest entry. Date to be corrected by: April 3, 2023.
2. Ensure pooling water is mopped. Pooling liquid may attract and promote breeding of pests (e.g. flies). Date to be corrected by: Today.
Note: Flies observed near back area where standing water is located.

Observation: Sanitizer dispenser located in dish pit area is dispensing Sink and Surface sanitizer at a concentration above the required range (see violation 209).
Corrective Action(s): Have dispenser adjusted / serviced so that it is dispensing Sink and Surface sanitizer within the required concentration range, as noted in violation 209. Date to be corrected by: March 28, 2023.
Note: This sanitizer dispenser is not to be used until such time; sanitizer dispenser located near walk-in cooler to be used instead in the interim.

Food is covered, and prepped items are labeled and dated. Raw animal products and ready-to-eat items properly organized and separated. Proper cooling and thawing practices in use.
Thermometers available for cold holding units. Probe thermometers available for internal food temperature checks.
Hand sinks are accessible and supplied with hot and cold running water, liquid soap, and paper towels.
Ice scoops stored outside of ice bins/machine. Cook line and ventilation equipment in sanitary condition.
Bar dipper well in use.
Sink and surface sanitizer measured within required range from dispenser located near walk-in cooler (required range: 272 to 700 ppm DDBSA, 704 to 1875 ppm lactic acid).
Main dishwasher measured 100 ppm chlorine in the rinse residual at the utensil surface.
Glass dishwasher (kitchen area) measured 50 ppm chlorine in the rinse residual at the utensil surface.
Bar glasswasher measured 12.5 to 25 ppm iodine in the rinse residual at the utensil surface.
Temperature and sanitation logs maintained and up-to-date.
Premises serviced by pest control technician. Most recent report dated March 21, 2023 notes "no pest issues." However, some limited small fly activity observed (ice machine area). Ice machine is covered.
