Observation (CORRECTED DURING INSPECTION):
REPEAT VIOLATION: No handsoap and single use disposable paper towel at one handsink in the kitchen. The other handsink has blocked soap dispenser.
Corrective Action(s):
Ensure to have every handsink fully equipped with liquid handsoap and paper towels in working dispensers at all times. Operator corrected these items at the time of inspection.

Observation:
- Observed notable amount of cockroaches caught on glue boards inside side cabinet attached to the freezer unit. Some live/free cockroaches present inside the cabinet as well. Cabinet was not used for food storage.
Corrective Action(s):
- Remove glue boards that are saturated. CLEAN and SANITIZE all surfaces of the cabinet. Pull out all items from the cabinets and thoroughly clean & sanitize. Every single storage compartment and cabinets will be checked during the follow up inspection. Staff started cleaning the cabinet at the time of inspection.

- Grease observed underneath cooking units.
- Food debris/sticky containers and trays observed for dry ingredients.
- Soiled metal storage racks.
- Rodent droppings observed on the floor of the electrical panel closet.
Corrective Action(s):
- Clean the areas mentioned above.
- Equip new containers and trays for the ones that are too old.
- Sanitize areas with rodent droppings after cleaning.

- All coolers at 4C or below.
- All freezers at -18C or below.
- High temperature dishwasher reached 71C or above during the final rinse.
- Ice machine in sanitary condition. Scooper stored on a hanger next to the machine.
- Meat slicer in sanitary condition.
- Monthly pest control service by Terminix.

*Consult with the professional pest control company regarding cockroach issue at the premises this week. Provide a physical report describing actions taken during the inspection visit & recommendations made by the company to the undersigned on Feb 6, 2023.
