Observation: Temperature logs do not have the month written down. Today's temperatures were recorded under the wrong date. The logsheet has been marked with permanent marker at later dates so it appears like future dates have been filled out already.
Corrective Action(s): Get new temperature logsheets. Ensure that staff are filling in the month section and recording temperatures on the correct dates.
