Observation: -Operating permit fee for the deli food service department is outstanding in the sum of $150.00. Attempts to contact Loblaws corporate head office by FHA billing clerk have been unsuccessful.
Corrective Action(s): -Food-service departments in the supermarket (Bento Nouveau & Deli departments) are subject to annual operating permit fees in the sum of $150.00. Invoices are mailed out in April of each year (start of fiscal year) to inform owners/operators of the permit renewal fees. The operating permit fee was received for the Bento Nouveau sushi department, but not the Deli department.
*Please provide payment of overdue permit fee for the Deli department in the sum of $150.00 to Fraser Health promptly to avoid enforcement proceedings.
