Observation (CORRECTED DURING INSPECTION): Observed a container of cooked chicken karage (deep fried chicken). Internal temperature measured at 22C. This batch of chicken karage was discarded due to time (~4 hours) and temperature abuse.

Observation (CORRECTED DURING INSPECTION): (Repeat violation) Hand sink station next to the 2 compartment sink was blocked by a metal shelf. Metal shlef removed at this time.

Observation (CORRECTED DURING INSPECTION): Observed a container of flour to coat chicken and a container of chicken in batter stored directly on the floor. Containers relocated at this time.

304 - Premises not free of pests [s. 26(a)]
Observation: (Repeat violation) Heavy fruit fly activity noted near the hand washing station next to the 2 compartment sink. Small amount of fruit flies noted by the grease trap (opposite side of the kitchen) and dining area (observed on top of fried garlic containers).
Corrective Action(s): 1. Clean and sanitize all areas impacted by fruit fly activity.
2. Call the pest control company for an assessment. Follow their recommendations to rectify pest issues. Retain a copy of report for review.
3. Improve sanitation of the facility (details in violation section 305 and 306)

Observation: Remove the following items:
- Empty soda cans and beer bottles located on the hall way
- Pails of used oil and empty buckets with oil residue at the bottom

Observation: Sanitation issues noted in the following areas:
- Every walls (sauce and other food debris build up)
- Heavy grease build up by the cook line and ramen boil station
- Surface of grease trap
- Cabinet doors and ice machine surface at the front bar
- Food debris and rodent droppings in the dry storage area (soak droppings with 100 ppm bleach solution for 10-15 minutes before picking them up with paper towels)
- Area in between equipment (e.g., in between walk in cooler and 2 door cooler, etc.)
- Large pool of water under the dishwasher due to pipe leaking

Corrective Action(s): Clean and sanitize all areas described in the observation section.

Observation: Observed heavy water leaking from two pipes that are connected to the sink located next to the dishwasher.
Corrective Action(s): Plumber was contacted at the time of inspection. Pipes are to be serviced.

Observation: Sanitation plan was not available on site for review.
Corrective Action(s): Retain a copy of sanitation plan.
