Observation: Rodent droppings and cockroach droppings observed underneath the ice machine, dishwasher and in multiple areas of the floor in the dry storage room; live cockroaches were observed running around during time of inspection; dead cockroaches were observed under the dishwasher and on shelving racks in the dry storage room; both adult and baby cockroaches were observed in hard to reach areas.
Corrective Action(s): A Closure Order has been issued, immediately close the premises; perform a thorough deep clean of the premises from top to bottom; contact your pest control company to eradicate all cockroaches until there are no more living rockroaches; contact the district EHO after all cleaning and pest control are done for a follow up inspection in order to re-open. Date to be corrected by: Immediately
