208 - Foods obtained from unapproved sources [s. 11] Observation: 1) Injeera bread was being sold on-site. Operator indicated that she gets it from the owner of Mostafa Supermarket who makes it at home. 2) Dried fish is not labeled. Operator couldn't find the receipt, but she called the supplier and asked them to resend it. Corrective Action(s): 1) You cannot sell foods from unapproved sources. Remove the injeera bread from the shelves. Do not offer this for sale. Also remove all advertising for it. 2) Email me the receipt by the end of the day tomorrow as discussed. Violation Score: 9

Bathroom sink is fully equipped Some foods are repackaged into smaller bags. LABEL ALL FOOD ITEMS IN BAGS AS DISCUSSED.
