Observation (CORRECTED DURING INSPECTION): A tray of vegetable pakora observed to be prepared to be re-fried to order. Operator informed that vegetable pakora is partially cooked a day before prior to fully-cooking the day after. Observed vegetable pakora measured at 5.6C internal temperature.
Corrective Action(s): Operator voluntary discarded aforementioned items in accordance to facility Food Safety Plan.

Observation (CORRECTED DURING INSPECTION): Pre-portion yogurt dip observed to be stored in the beverage cooler holding an air ambient temperature of 14C over night. Container of yogurt, green sauce observed to be stored in the preparation cooler measured at 13C air ambient temperature located in the office room.
Corrective Action(s): Discard aforementioned items. All cold potentially hazardous foods must be stored at 4C or lower.

Observation (CORRECTED DURING INSPECTION): Can opener blade observed to have built-up of food debris.
Corrective Action(s): Clean and sanitize aforementioned item. Can opener blade must be cleaned and santiized after each-use.

Corrective Action(s): Prepare chlorine sanitizer buckets for the kitchen to measure between 100-200ppm chlorine.

Observation (CORRECTED DURING INSPECTION): Kitchen handwashing sink obstructed with garbage bin. Bar handwashing sink lack paper towels.
Corrective Action(s): Ensure all handwashing sinks are well equipped with hot and cold running water, liquid soap, and paper towels. All handwashing sink must be unobstructed to facilitate proper staff hand hygiene.

Observation: Observed raw chicken stored beside cooked chicken in the walk-in cooler and various foods are not covered in the walk-in cooler. Observed frozen meats in the walk-in freezer are exposed without lids or covers and are stored on the floor.
Corrective Action(s): Store all raw meats below all ready-to-eat foods. Store all foods at least 6 inches off the floor. Cover all food items. Corrected by: Today.

Observation: Built-up of food debris observed underneath and behind the following areas:
-Chest freezer in the kitchen.
-Cookline.
-Shelving units in the walk-in cooler and freezer.
-Beverage cooler in the office room.

Built-up grease observed on the floors of the walk-in cooler and exhaust vent hood.

Debris build-up observed along the wall of the dry spice shelving units.
Corrective Action(s): Clean and sanitize the aforementioned items to remove built-up of food debris, grease, and/or debris. Corrected by: June 27, 2022.

Observation: Beverage cooler and preparation cooler in the office room observed to be measured at 14C and 13C air ambient temperature. Chest freezer in the office room measured at -5.5C air ambient temperature.
Corrective Action(s): Service the aforementioned items to hold an air ambient temperature of 4C or lower. Do not use the cold-holding units until it is able to hold an air ambient temperature of 4C or lower. Service the chest freezer in the office room to hold an air ambient temperature of -18C or lower. Corrected by: June 27, 2022.

Walk-in coolers and preparation cooler in the kitchen measured at 4C. Walk-in freezer and chest freezer in the kitchen measured at -18C.
Low-temperature dishwasher measured at 50ppm chlorine.

NOTE: Notify any changes to floorplan, menu, or services to district inspector for approval. No changes are permitted without health approval. Review facility's Food Safety Plan and Sanitation Plan to staff. Ensure sanitizer buckets are available at all times in the kitchen and front servicing area at all times to permit sanitation of food contact surfaces.
