Observation: Observed premises not free of pests; fruit flies/flies present in excessive amount near back door, and dishwashing area. Also present in fewer amount throughout other areas (bar area and near drains (mop sink).
Corrective Action(s): Ensure that premises is free of pests to ensure premises is maintained in a sanitary condition.

Observation: Observed areas in the premises not maintained in sanitary condition;
1. top of dishwasher and surrounding area,
2. Floors in walk-in cooler & freezer (build-up on crates and ice),
3. inside of ice machine,
4. Area around dough machine,
5. Ceiling tiles with dust/build-up,
6. Water in coolers and under sink from leaks,
7. Dishware carts - old cardboard and build up.
Corrective Action(s): Ensure premises is cleaned regularily and maintained in a sanitary condition.

Observation: Observed equipment not in working order;
1. Front kitchen hand washing sink leaking, desert and pizza prep (built) coolers with leaking water inside.
2. Gaskets on salad prep line cooler.
Corrective Action(s): Ensure that equipment is fixed/repaired and maintained in working order; fix leaks to prevent further leaking and potential contamination of surfaces and food.
Ensure that gasket on salad cooler is replaced to allow for proper seal when closed and to maintain temperature.

Observation: Observed a few floor tiles surrounding drain infront of dishwasher area to be missing with pooling water present.
