Observation: - Storage shelves and vegetable cutting equipment is dirty (food debris).
- Ice maker had black mold inside and food debris outside.
- Few mice droppings noted on the dry storage shelf, which a site of the walk-in unit.
Corrective Action(s): - All equipment and food contact surfaces must be kept clean.
- Clean and sanitize the ice maker.
- Clean and sanitize the contaminated shelf.

Observation: - Dishwasher fount at 0.0ppm of chlorine at the time of inspection.
Corrective Action(s): - Repair/service the dishwasher.
Note: The operator on site conducted a telephone call to service the dishwasher.

Observation: - Multiple food items are not covered in the prep line coolers & walk-in cooler.
- Potato and onion bags stored at the floor at the time of inspection.
- Sticky traps observed on the kitchen ceiling.
- Two yellow cutting boards stored behind the prep table on the floor.
Corrective Action(s): - Today. Ensure all food items are covered.
- All food must be stored off the floor.
- Remove sticky traps and address flies issue to a pest service.
- Keep cutting boards in a sanitary manner/all equipment and utensils must be stored properly (eg., on shelves)/off the floor.

Observation: Two mice found in a box mice trap. The trap located under the prep table, which close to the dishwasher.
Corrective Action(s): Contact your pests service and ask for service. The operator stated that service has been done a week ago.

Observation (CORRECTED DURING INSPECTION): A back door has been open at the time of the inspection
Corrective Action(s): Keep the back door closed all the time. It is pests issue already.

Observation: Card board (in bar cooler) & news paper covered shelves. Some news paper covers are very dirty and oily.
Corrective Action(s): Remove all card boards and news paper. Food contact surfaces must be durable, food grade, and easy to clean and sanitize.

Observation: Dishwasher is not working properly. No sanitizer in the cavity.

Comments

- General sanitation is acceptable but need to be improved.
- Handsinks (one in kitchen and one in bar area) supplied with liquid soap, paper towels, hot/cold running water.
- Prep cooler across cooking line at 3.4 deg C.
- Counter top cooler at 3.5 deg C.
- Upright cooler at 3.0 deg C.
- Walk in cooler: 3.6 deg C.
- Walk in freezer: -18 deg C.
Note: Glass door cooler, which has been in a small storage room front of the cooking line removed from the premise (was broken). The operator claimed that they are looking for a different new cooler.
- Ice scoops (front and kitchen) stored safely at the time of inspection.
Note: Take out and single use utensils only for now. The business is slow at the time of inspection. The operator stated that the service will be coming at 3:30 pm. Follow up inspection will be conducted. If the dishwasher remain broken the Closure Order will be issued.
