Observation: Cockroach activity noted in glue traps placed underneath stairway in the kitchen that leads to the second level.
Cockroach activity also in glue traps noted in the upstairs for dry storage of food.
Corrective Action(s): Operator presented monthly invoices from pest company for their service. However, the pest report does not include information such as: the level of cockroach problem and the remediation actions taken. Placing glue traps throughout the kitchen and storage area is insufficient as this is only a monitoring action and does not remediate the cockroach problem. Operator claims to be in the process of hiring a new pest control company to perform the remediation treatments.
Cockroach problem must improve during follow-up inspection. Failure to do so may result in further enforcement actions such as closure or issuance of violation tickets.

Remove all unnecessary cardboard boxes
All shelving units and equipment in the upstairs area and kitchen area need to be moved 6 inches away from the wall for cleaning purposes and for identify potential entry points.
