Observation: 1. Debris observed on interior bottom surface of ice bin located at bar.
2. Mildew/biofilm observed on some interior surfaces of ice machine.
Corrective Action(s): 1. Ice bin was emptied to be washed and sanitized at time of inspection. Ensure that ice bin is being cleaned and sanitized as per your sanitation plan. *CORRECTED*
2. Ensure ice machine is properly cleaned to remove all midew/biofilm, then sanitized. Discard ice prior to cleaning and sanitizing. Date to be corrected by: February 29, 2019.

Observation (CORRECTED DURING INSPECTION): Clean dishes and bottles of oil/vinegar stored directly under soap dispenser unit in pizza preparation area.
Corrective Action(s): Above-noted items were removed from underneath the soap dispenser. Ensure that no food/equipment/utensils are stored in this area as water/soap from hands may potentially drip onto these items.

Observation: Food debris/spills observed in the following areas:
1. Under bar counter where ice bin and hand sink are located
2. Door seals of coolers
Corrective Action(s): Ensure that the above-noted areas are properly cleaned. Ensure these areas, including any hard to reach areas, are being cleaned routinely to limit build up of debris. Note: Food debris may potentially attract pest activity (e.g. syrup may attract flies). Date to be corrected by: February 29, 2019.

Observation: Small service area cooler was measured at 9 degrees C. Thermometer located within the cooler was indicating freezing temperatures. Note: Limited food storage (small amount of non-perishables and cream). Temperature of cream: 9 degrees C.
Corrective Action(s): Cream discarded. Ensure that no potentially hazardous foods (perishables) are stored in this cooler until it is capable of maintaining 4 degrees C or less. Have cooler adjusted or serviced as needed. A different thermometer was placed into the unit at the time of inspection. Date to be corrected by: March 8, 2019.

- Hand washing stations are easily accessible and supplied with hot and cold running water, liquid hand soap, and single use paper towels.
- Staff observed to be properly washing hands at time of inspection.

- Low temperature dishwasher measured 50 degrees C and 100 ppm chlorine in the rinse residual at the utensil surface
- Bar glass washer measured 12.5 to 25 ppm iodine (required: 12.5 to 25 ppm iodine)
- Quaternary ammonium compounds (quats) sanitizer available at 200 ppm in labeled spray bottles and from dispensing unit
- Washrooms maintained in sanitary condition
- Shellfish tags kept on site
*Exception: Small server station cooler at 9 degrees C (refer to violations).
Note: Significant accumulation of ice observed in upper frozen storage compartment of small service station cooler. Defrost and clean.
