Observation (CORRECTED DURING INSPECTION): Black moldy and mildew noted on wooden serving boards for sushi products. Wooden boards are starting to get worn and chipped.
Mildew noted on the condensing tray inside of the ice machine.
Clean knives were stored underneath the front hand washing station.
Corrective Action(s): All moldy and worn wooden boards were identified by operator and are to be discarded.
All ice products inside the ice machine are to be discarded and unit is to be properly washed and sanitized.
Ensure all knives are stored in a sanitary manner after they are washed and sanitized. If they are not going to be used, they should be removed from the premises.

Observation (CORRECTED DURING INSPECTION): Wiping rags were stored directly on cutting boards. No sanitizing solution was available in the back kitchen.
Observed staff washing a large sushi mixing bowl in the mop sink.
Corrective Action(s): Ensure all wiping rags are stored in a 100ppm chlorine solution.
Mop sink is for mop water only and not intended to wash and sanitize equipment or dishware. Sushi bowl was re-washed and sanitized in the 3-compartment sink.

Observation: Build up of fruit flies noted in the mop sink area.
Corrective Action(s): Mop bucket was removed from kitchen. Ensure to take the necessary steps to eliminate and prevent the fruit fly problem. A professional pest control company should be contracted to prevent pest problem.

Observation: Old accumulation of food debris, grease, and heavy black dirt was noted around hard to reach areas such as: area by dishwasher, mop sink, food preparation table, and front prep cooler.
Corrective Action(s): Premises requires a thorough cleaning.

Routine inspection conducted and the following were noted:
Chlorine dishwasher achieved a final rinse concentration of 100ppm. Chlorine test strips available. Ensure unit is tested daily with chlorine test strips to ensure it is in good working work.
