Observation: Mouse droppings noted throughout the premises.
Corrective Action(s): Step up your pest control program. Identify harborage areas, seal off access, store bags of flour and rice in hard containers with lids. Do not disturb traps when cleaning the floor.

Observation: Droppings noted on top of the dishwasher, on top of lids, stored items, and on floors...etc. Soiled clothes stored in close proximity to food items inside the refrigered make table.
Corrective Action(s): Remove droppings, sanitize surfaces, organize storage and remove clutter. Soiled clothes have been removed.

Thoroughly wipe down all food contact surfaces in the morning after arriving.

Observation: No sanitizer detected in the dishwasher after running several times.
Corrective Action(s): Service the dishwasher. Sanitize dishes after they are washed in the dishwasher by immersing in a sink of sanitizer solution ( 1/2 oz bleach per gallon water)

At least one of the spray bottles of chemicals were in correctly labeled. Ensure they are clearly marked, e.g., bleach sanitizer for food contact surface, quats sanitizer for customer tables...etc.
