Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation: Dishwasher did not dispense any detectable chlorine.
Correction: Repair dishwasher. Until it is repaired, wash dishware and equipment in the dishwasher and MANUALLY sanitize. After the wash cycle, place all equipment in a 200ppm bleach solution for 2 minutes before allowing to air dry.
Comments

- Dishwasher has been repaired, measured 50ppm chlorine at the final rinse cycle
- continue to wash equipment right BEFORE use to ensure all contamination is removed
- clean up rodent droppings as soon as they are found, using a 1:10 bleach solution (1 cup bleach: 10 cups water)
- seal any holes, cover all foods
- follow all recommendations by pest control
