HAZARD RATING FOR YOUR FACILITY: Moderate (16-29) Total Score = 28
Critical Hazards: Total Number: 2
302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation (CORRECTED DURING INSPECTION): 1) No detectable chlorine residual in the low-temp dishwasher at onset of inspection. Sanitizer supply bucket was recently changed out, but the sanitizer feed tubing wasn't correctly submerged in the sanitizer liquid, resulting in the dishwasher pump only pumping air during the rinse cycle.
2) Low quats sanitizer concentration in the spray bottles (ie. <200ppm).
Corrective Action(s): 1) Remind all kitchen staff on correct maintenance procedures for the dishwashing machine. Whenever the dishwasher chemical(s) are changed, the feed tubing must be fully-submersed in the liquid. After installing the new sanitizer pail, the feed pump must be primed to purge air from the line. Confirm by holding the primer switch until a steady stream of liquid is observed being dispensed into the dishwasher.
2) Replace sanitizer solution in all of the sanitizer spray bottles with fresh solution
Ensure to run the dispenser for several seconds before initially filling the spray bottles to ensure chemical concentrate is reaching the solution.
Confirm sanitizer concentrations using test strips provided by your supplier.
Violation Score: 5

402 - Employee does not wash hands properly or at adequate frequency [s. 21(3)]
Observation (CORRECTED DURING INSPECTION): No paper towels available at the hand-washing station in the bar. Staff indicate that they use bar towels to dry hands.
Corrective Action(s): *Operator instructed to place paper towels (or napkins) near the hand-sink as a temporary corrective measure.
*Do not use cloth towels to dry hands, as they can re-contaminate hands after washing (through repeated use)
*Install a paper towel dispenser in the bar near the hand-washing sink
Violation Score: 5

Non-Critical Hazards: Total Number: 2
304 - Premises not free of pests [s. 26(a)]
Observation: Ongoing rodent activity in the premises.
Corrective Action(s): Continue pest-abatement efforts by following recommendations from pest control technicians and establishing a thorough cleaning schedule for the premises.
Violation Score: 9

306 - Food premises not maintained in a sanitary condition [s. 17(1)]
Observation: Rodent droppings observed on lower shelves in the pantry and on floor perimeters throughout the premises.
Corrective Action(s): *Remove rodent droppings and sanitize surfaces where droppings are found.
*Maintain a high-standard for regular, frequent cleaning of surfaces throughout the premises, especially areas where foods are stored, handled, prepared.
Violation Score: 9
