203 - Food not cooled in an acceptable manner [s. 12(a)]
Observation (CORRECTED DURING INSPECTION): (1) Discussed cooling procedure with owner/operator who stated that once the sauces finished cooking stay at room temperature for less than two hours (good) and are then transferred to large buckets around half full.
(2) Observed cooked penne in deep insert in line cooler at 20C internally. Operator stated that it was just cooked and placed in the insert for future use.
Corrective Action(s): (1&2) All food must be cooled to room temperature within 2 hours then from room temperature to less than 4C within four hours. To achieve this, food must put into shallow containers, small portions or an ice bath.

301 - Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
Observation: Meat slicer downstairs found with dried on food debris. Operator stated that this is no longer in use.
Corrective Action(s): Thoroughly clean and sanitize meat slicer and store in sanitary condition so that it can be used without cleaning if need be. Alternatively if this is no longer needed for operation it can be removed from site. Cleaning due by tomorrow.

Observation (CORRECTED DURING INSPECTION): Observed a sealed package of previously frozen raw shrimp stored on the counter.
Corrective Action(s): Food must be thawed in the cooler overnight or in cold water.

304 - Premises not free of pests [s. 26(a)]
Observation: Three rodent droppings found on floor between food plate assembly area and cooler to the left. Bait box found behind dishwasher.
Corrective Action(s): Bait box removed during inspection, no bait can be used inside the restaurant for pest control. Remove droppings, clean and sanitize area and monitor for more activity. Pest control company may need to be called in if signs of pests continue.

Observation: Build-up of dust, food debris and cob webs in several areas in premise.
Corrective Action(s): Premise requires a deep clean. The areas of the premise that require cleaning include (but are not limited to):
- cob webs in ceiling of basement
- floor level commercial mixer exterior
- under grills and cooking equipment
- general dusting in kitchen
- ice machine water reserviour

Observation: Several pieces of equipment found severely worn, broken or missing including:
- butcher block cutting board near food/order assembly line severely worn and no longer easily cleanable
- butcher block cutting board near salad assembly line severely worn and no longer easily cleanable
- counter top downstairs in food prep area where meat slicer was also found severely worn and no longer easily cleanable
- chest freezer lid in chest freezer downstairs in corner opposite meat slicer and floor stand mixer
- light bulb covers missing in all light fixtures upstairs and down
- one light ballast broken above stock pot burner
- raw wood strip along back of line cooler in food assembly area
- cardboard found lining shelves and equipement
Corrective Action(s): Remove cardboard and repair and/or replace these surfaces with level, food grade, durable, and easily cleanable ones. Due: Oct 24th except for butcher block cutting board at salad bar cooler, operator requested a longer time, one month agreed upon, due Nov 10th, 2019.

Observation: Three bottles of what the operator identified as quats sanitizer were found without labels.
Corrective Action(s): Label all chemicals with bottle contents so staff can quickly and accurately identify contents.
