Observation (CORRECTED DURING INSPECTION): Bag containing rice appeared to be chewed through one corner.
Pest droppings observed within some dry food storage containers with no lids (e.g. single packaged condiments).
Corrective Action(s): Above-noted items discarded by operator. Ensure potentially contaminated food is immediately discarded. Ensure food is stored in appropriate pest-proof containers with lids.

Observation (CORRECTED DURING INSPECTION): Left preparation cooler measured approx. 7 to 8 degrees C. Note: Cooler was measured at 4 degrees C or less yesterday.
Corrective Action(s): Cooler may be on defrost; monitor and adjust or repair as needed to ensure food is maintained at temperatures of 4 degrees C or less. All food was removed from cooler as a precaution and placed into another cooler at 4 degrees C or less.

Observation (CORRECTED DURING INSPECTION): Pest droppings observed in some utensils/equipment storage bins.
Open bags of single use utensils (e.g. plastic forks/knives) observed. Pest droppings observed.
Corrective Action(s): Items moved to dish pit to be washed, rinsed, and sanitized. ALL equipment, utensils, and food contact surfaces on the premises must be properly washed, rinsed, and sanitized.
Single use utensils discarded. Ensure going forward, all single use utensils, napkins, etc. are stored in pest-proof containers with lids.

Corrective Action(s): ALL rodent droppings to be removed. Ensure all surfaces (including but not limited to floors, floor-to-wall junctions--including areas behind equipment, all shelves, equipment, utensils, and countertops) must be properly cleaned then sanitized. Observation: 1. Hole in wall observed under back area hand sink. Hole observed to be sealed with cloth.
2. Leak observed under dishwasher. Bus pan in place to catch leaking water.
Corrective Action(s): 1. Ensure all holes/gaps that may potentially allow for pest entry are properly sealed. Discuss with pest control. Cloth is not effective at preventing pest entry. Date to be corrected by: Today.
2. Empty bus pan today to remove standing water. Have above-noted leak repaired. Standing/leaking water may attract pest activity. Date to be corrected by: July 13, 2021.

Correction: Ensure hot water is available for premises at a sufficient temperature and pressure at all times. Hot water must be available at all hand sinks at a minimum temperature of 38 degrees Celsius for proper hand washing. Date to be corrected by: Today.

Employee does not wash hands properly or at adequate frequency [s. 21(3)]
Observation: Staff member observed touching face/mask without properly washing hands afterwards. *CORRECTED*
Adequate hot water not available for proper hand washing.
Correction: Ensure all staff are properly washing hands with hot and cold running water, liquid hand soap, followed by drying with single use paper towels as often as needed to prevent contamination of food and food equipment/utensils/surfaces. For example, hands should be properly washed after touching clothing, hair, skin.

Hot water temperature was measured at 49 degrees C.
High temperature dishwasher was re-checked; temperature measured 76 degrees C at the utensil surface.
Coolers at 4 degrees C or less.*
Freezer at -16 degrees C.
Quats available at 200 ppm in bucket with wiping cloths.
Hand washings sinks unobstructed and supplied with hot and cold running water, liquid hand soap, and single use paper towels.
