Report of Findings
Licensing received a complaint on July 9th, 2019, regarding care and supervision, policies & procedures, physical facility, and records & reporting.

The allegations included the following details:
- The complainant was not notified of an incident
- Care needs of the person in care (PIC) were not met
- Equipment and furnishings were not maintained in a safe and/or clean condition

All areas of the complaint were fully investigated. Evidence was obtained through person in care’s records, facility policies, correspondence, inspection and photographs.
The following contraventions were determined:
· Residential Care Regulation Section 85 (1)(d) - Policies and procedures
· Residential Care Regulation Section 21 (d) - Equipment and furnishings
· Residential Care Regulation Section 77 (2)(c) - Reportable Incidents

Observed Violations/Contraventions: A summary of the violations found during the complaint investigation are listed below.

31270 - RCR s.21(d)A licensee must ensure that all furniture and equipment for use by persons in care (d) are maintained in a safe and clean condition.
Observation: Multiple areas of the facility were found to be inadequately maintained and presented as unclean or in need of repair.
Corrective action: Corrective action plan required.
33280 - RCR s.85(1)(d)A licensee must do all of the following: (d) ensure that policies are implemented by employees.
Observation: Staff did not transfer the PIC as per facility policy.
Corrective action: Corrective action plan required.
39090 - RCR s.77(2)(c)Subject to subsection (3), if a person in care is involved in a reportable incident, the licensee must immediately notify (c) a medical health officer, in the form and in the manner required by the medical health officer.
Observation: The facility staff failed to notify a medical health officer that a person in care was involved in a reportable incident.
