Observation: 1. Floor tiles appear to be missing in certain areas of the food premises (e.g. floor beneath and behind the dispensing equipment in the front service area).
2. Floor liner is partly broken in the dry storage room near the walk-in-freezer.
3. Some floor tiles are broken in the kitchen area.
Corrective Action(s): Ensure to replace the broken floor tiles, missing floor tiles and broken floor liner with intact floor tiles; correct within 3 months.

If renovations with a different flooring material are proposed, submit a finishes schedule to the Environmental Health Officer for review and health approval prior to making the changes. Flooring materials are required to have finishes that are smooth, easy to clean, durable, non-toxic, and impervious to moisture.

Follow up inspection:
-200 ppm QUATS sanitizer was available in the sanitizer pails, both spray bottles, and dispenser at the 3-compartment sink.
-QUATS test strips were available on-site.
