Observation: Multiple house flies observed on site during time of inspection in food preparation and dishwashing area. One UV light observed placed on site at the back of the premise near the receiving door.
Corrective Action(s): Place sticky fly traps around non-food preparation areas to reduce current house fly population. Monitor house fly population. Contact pest control company to assess and place traps as necessary. Keep all doors and windows closed to prevent fly entry on site. Ensure all food is protected from potential contamination from flies by keeping foods covered and at least 6 inches off the floor.

Observation: Food premise is not observed in a sanitary manner. The following areas are observed unsanitary:
- Insides of all coolers (including cooler seals on doors and drawers)
- Cooler handles
- Floors around and behind all cooking units
- Floors around and behind preparation table
Corrective Action(s): Degrease, clean and sanitize the above mentioned area to remove grease and food debris and to prevent pest activity. Clean and sanitize the floors more frequently (as per approved sanitation plan) to ensure the premise is maintained in good sanitary condition.

Observation:
1. Clean and sanitized equipment observed stored next to dishwashing area (within 2-3 feet) on a wire shelf. Equipment is not protected from potential contamination from dirty dishware from the 3-compartment sink.

2. Single-use disposable dishes and take-out containers observed stored at back of premise on a wire shelf close to restrooms. Single-use containers are not covered or protected from potential contamination from customers.
Corrective Action(s): Propose adequate, sanitary storage for all clean and sanitized equipment and single-use disposable containers. Submit plans to the health inspector for review and re-inspection.

- Temperature log maintained up to Aug 12, 2022
- General sanitation okay during inspection (Refer to violations)
- Hand washing stations are supplied with hot and cold running water, liquid soap and paper towel
- QUATs sanitizer in spray bottle (X1) measured at 200 ppm QUATs concentration with test strip
- High-temperature dishwasher measured at 74.6'C at plate surface (above minimum required final rinse temperature of 71'C)
- 3-compartment sink on site with drain plugs
- All food observed stored at least 6 inches off the floor and covered
- FOODSAFE Level 1 trained staff available on site during time of inspection
- Health permit posted in conspicuous location on site
