HAZARD RATING FOR YOUR FACILITY: Low (≤15) Total Score = 11
Critical Hazards: Total Number: 1
302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation (CORRECTED DURING INSPECTION): 1. Bleach water sanitizer solution in spray bottle at front measured at greater than 200 ppm chlorine (hypochlorite)
2. Bleach water sanitizer solution in spray bottle at back (kitchen) had an undetectable level of chlorine (hypochlorite)
Corrective Action(s): Above-noted sanitizer solutions were re-made and tested at 100 to 200 ppm chlorine (hypochlorite). Ensure that bleach water solution that is intended to be used for food contact surfaces (e.g. counter tops) is prepared at 100 to 200 ppm chlorine to allow for effective sanitizing. Higher concentrations may potentially lead to contamination of food. Ensure that all spray bottles are properly labeled to identify contents.
Violation Score: 5

Non-Critical Hazards: Total Number: 2
209 - Food not protected from contamination [s. 12(a)]
Observation (CORRECTED DURING INSPECTION): An open container of single use utensils and containers was located under the paper towel dispenser at the front hand sink.
Corrective Action(s): Single use utensils and containers were relocated away from the hand sink. Ensure no food or utensils are stored near the hand sink. Water from hand washing may potentially splash or drip onto these items if they are located in close proximity to the hand sink.
Violation Score: 3

304 - Premises not free of pests [s. 26(a)]
Observation: Approximately five to ten live ants were observed at floor-to-wall junction located near kitchen hand sink.
Corrective Action(s): Ensure that ant population is controlled. Contact a licensed pest control operator if problem persists. Note: Remove any potential sources of food and continue to maintain the premises in sanitary condition. Date to be corrected by: April 21, 2019.
Violation Score: 3

Follow Up to "Critical" Violations Noted on Previous Inspections (if applicable): No corrections entered
