Observation (CORRECTED DURING INSPECTION): Observed stacked dishes, cups, bowls and utensils improperly organized in the domestic washer.

-utensils too tightly packed in holder
Corrective Action(s): You must organize the cups/bowls/plates in ONE LAYER in the slots provided and do not overly pack the utensil washer holder as those ware inbetween will not be adequately cleaned and sanitized.

re-wash all the ware in the dishwasher
