Introduction

An unscheduled routine inspection was conducted in the presence of the Manager to assess compliance with the Community Care and Assisted Living Act (CCALA), the Residential Care Regulation (RCR), and the relevant Director of Licensing Standards of Practice (DLSP). Evidence for this report was based on the licensing officer’s observations, review of the facility records, and information provided by the facility staff at the time of inspection.
The following areas were reviewed:
- Licensing
- Physical Facility
- Staffing
- Policies and Procedures
- Care and Supervision
- Hygiene and Communicable Disease Control
- Medication
- Nutrition and Food Services
- Program
- Records and Reporting
As part of this routine inspection, a facility risk assessment tool is completed and a copy is provided. The risk assessment includes non-compliance identified during the routine inspection, and a 3-year historical review of the facility’s compliance and operation.

Current Inspection - Items reviewed comply with the Act, regulations & standards of practice except for those noted on supplementary pages.

Observed Violations
PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31290 - RCR s.22(1)(b) - A licensee must ensure that all rooms and common areas are (b) maintained in a good state of repair.
Observation: When entering the home is a living area with a ottoman in the middle surrounded by sofas. The ottoman is showing wear and tear around the top edges whereby the ottoman material has come off making it hard to clean as well. The writer problem solved with the Manager as to options to consider such as possibly purchasing a cover that is cleanable to cover the ottoman.
Corrective Action(s): Please ensure all rooms and common arears are maintained in a good state of repair throughout the entire home (inside and outside).
Date to be Corrected: Please provide a written response to this item by the timeline noted in this report.

CARE AND/OR SUPERVISION: 34760 - RCR s.81(4)(b)(ii) - A licensee must ensure that (b) each care plan is reviewed and, if necessary, modified (ii) if there is no substantial change in the circumstances of the person in care, at least once each year to ensure it continues to meet the needs and preferences, and is compatible with the abilities, of the person in care who is the subject of the care plan.
Observation: A review of a person in care's chart indicated the personal support plan was dated January 2020. The Manager confirmed this personal support plan needs to be reviewed.

In addition, a review of a person in care's chart indicated the following:

- Nutrition Care Plan Summary to be reviewed February 2021, but hasn't been reviewed.
- Nutrition Survey to be reviewed February 2021, but hasn't been reviewed.
- Screening form when to involve a Dietitian to be reviewed February 2021, but hasn't been reviewed.
- Food and Nutrition Information to be reviewed February 2021, but hasn't been reviewed.
Corrective Action(s): Please ensure care plans are reviewed at least once a year if there is no substantial change. as per section 81 (4)(b)(ii) of the Residential Care Regulation.
Date to be Corrected: Please provide a written response by the timeline noted in this report.

RECORDS AND REPORTING: 39090 - RCR s.77(2)(c) - Subject to subsection (3), if a person in care is involved in a reportable incident, the licensee must immediately notify (c) a medical health officer, in the form and in the manner required by the medical health officer.
Observation: The Manager advised the writer at the time of the inspection there were positve cases involving persons in care back in January 2021. The Manager stated the funder was sent a report, but Community Care Facilities Licensing was not sent a incident report. The writer stated when there is one positive covid-19 case involving a person in care, then a reportable incident report for "Disease Outbreak" is to be submitted.
Corrective Action(s): Please submit the Disease Outbreak incident report to Community Care Facilties Licensing.

Printed staff checklist:
- The checklist makes reference to medication training not being documented for a staff member. The Manager stated this will be looked into. Please let the writer know the outcome of this after you have looked into this as part of the written response to this routine inspection report.
Due to the Covid-19 pandemic, the findings were reviewed with the Manager at the time of the inspection. This inspection report, and risk assessment was written off-site and then emailed on March 12, 2021 to the Manager for review and to finalize the report and risk assessment once they were in agreement to the wording. As a result of the pandemic, signature for the Manager is not included. If there are further questions related to this routine inspection, please contact your Licensing Officer.
