Observation: Rodent droppings are observed on floor by back wall, floor next to hot water tank, beside and behind large chest freezer. Staff indicated pest control company visits once a month.
Corrective Action(s): Remove droppings, sanitize areas, and notify pest control of areas where droppings are noted. Follow any recommendations pest control technician provides.

Observation: 1. A hole is observed at the bottome of the wall under shevling rack (by back door).
2. Ceiling tile is missing above walk-in cooler, and another next to it is not in place properly.
Corrective Action(s): As facility is dealing with pest problem, all possible entry points must be sealed and closed off. Seal hole, and close ceiling tiles.
