Observation: One sandwich prep cooler not properly cold holding. Temperature at 8c. All PHF's relocated.
Corrective Action(s): Do no use until repaired and is capable of cold holding at <4c

Observation: Wiping rags observed being stored at workstations when not in use.
Corrective Action(s): Wiping rags when not in use MUST be stored in a sanitizer container and kept fully submerged. Solution is to be changed every 2 hours or more frequently depending on the organic load introduced into the sanitizer.

Observation: Mice droppings observed in premises.
Corrective Action(s): Bins purchased but NO LIDS observed to prevent rodents from climbing into the bins. Bins containing food items MUST be LIDDED when not in use.
Purchase glue boards to be deployed at night.

Observation: Openings not sealed at base of back door and front door . These openings are sufficient to allow the entrance of pests/rodents
Corrective Action(s): Affix weather stripping to prevent pest entry.

Observation: Rodent droppings still observed below and behind all kitchen equipment.
Corrective Action(s): Thoroughly clean and sanitize ALL floor surfaces throughout the kitchen and food storage areas. This will entail pulling out equipment in order to reach the floor/wall surfaces, shelving surfaces in the kitchen.

Observation: Paper towel dispenser observed to be not working properly at hand sink opposite to dishwasher
One Fluoresent light in ceiling fixture observed to be not working.
News paper and cardboard observed being used in the kitchen. These were removed as instructed at time of inspection
Corrective Action(s): Repair paper towel dispenser.
Replace burned out fluoresent light
Never use cardboard or newspaper in the kitchen on the floor or shelving.
