Observation:
1) Live cockroaches noted in glue traps.
2) Rodent droppings noted:
- inside walk-in cooler
- on top of hot water heater
- on top of prep surfaces in back area
- along the walls behind equipment
- near staff belongings

Pest report is not descriptive and only states that glueboards were changed. Staff stated that the pest control technician was asked to control for cockroaches as well but pest report only has "rodents" marked off.

Corrective Action(s):
1) Clean and sanitize entire facility, including customer dining area. Use a 1:10 bleach solution.
2) Seal all holes.
3) Contract a different pest control technician if the current one is unable to control the population of cockroaches and rodents.
4) Place all foods in pest proof containers.

General:
- handwashing station stocked with soap, paper towels and hot and cold running water
- sanitizer measured 200ppm quats in spray bottle
- dishwasher dispensed 50ppm chlorine at final rinse cycle

Note: "Bush" brand orange red powder (IH 7802) is HELD until inspector can verify use in food. Do not use, sell, giveaway or remove from facility.
