Observation: Gaps underneath the counters and around the pipes underneath the sink allow for cockroach harborage. These need to be eliminated as removing cockroach harborage points is part of the eradication process. Pest control provider should continue to look for structural and sanitation issues that support cockroach activity.

-Followup Inspection
-Closure order has been rescinded
-Pest control provider came to service on Oct 25, 2019. A proper monitoring program must be in place immediately. This is to measure the level of cockroach activity.
-One dead cockroach underneath the sink. No live cockroaches found.
-Display coolers are maintaing 4C or less. Continue to monitor the temperatures and take corrective action as required. Either repair or replace cooler if it malfunctions again.
-All previously noted areas have been cleaned and sanitized and no cockroaches noted in those areas.
-Pest control provider service should be conducted weekly for 1 month until the cockroach situation is confirmed to be under control before moving to a service frequency of once per month.
