Observation: - Adult and baby cockroach observed on glueboard inside rodent trap.
Corrective Action(s): - Thoroughly clean premises (See Violation 306).
- Store all items at least 6" (15 cm) off the ground to facilitate cleaning and pest control monitoring.
- Request for cockroach treatment from pest control company.
- Ensure all areas of the premises are accessible for pest control service.
- Continue to monitor pest activity and take corrective action if new and/ or increased pest activity is noted.

- Dirt and food debris accumulation under front service counter equipment.
- Food debris accumulation inside rice pot wells.

- Handsinks are accessible and provided with hot and cold water, liquid soap and papertowels.
- Cold Holding Equipment are maintaining food at acceptable temperatures (≤4C).
- Freezers are maintaining food at acceptable temperatures (-18C).
- Hot Holding Equipment are maintaining food at acceptable temperatures (≥60C).
- Food storage practices appear satisfactory. Food is protected from contamination.
- Interior of ice machine and ice storage bin appears maintained in a sanitary condition.
- Wiping cloths stored in 200 ppm QUATS.
- Manual 3-Compartment sink washing and sanitizing procedures in use. 200 ppm QUATS used for sanitizing food equipment and utensils.
