A consultation inspection was conducted due to receipt of a complaint that PHO Order re: PoV verification not being done, facility was getting ready to open ( visit a bit prior to Business hours) the following observations were made:

- Spoke to chef about the PHO order regarfding PoV verification. Staff admitted that the facility was not checking PoV before but after getting fine they are checking. Staff asked :
-if customres come in and PoV is checked upone arrival but if they go out for smoke and come back, what to do? Explained the PHO order, if a customer PoV is verified no need to check again when the customer left the facility for short time and come back.
-The business holds a valid liquor license. As per the Food and Liquor PHO Order (Oct. 25/21), all businesses that hold a valid liquor license is required to verify patrons' PoV, regardless of the business is doing table service or not.
- Staff must either visually verify the patrons' PoV card or scan the QR code, AND visually check another piece of government issued photo ID. The names on the PoV and government ID must match, the patron must be fully vaccinated, and the photo on the ID must be the same person presenting it.
- If a patron is only picking up a food order for take-out and no dine-in, you do NOT need to verify their PoV.
- It is highly recommended that a sign is posted on the front door informing patrons that PoV are required to be checked. This may help prevent disgruntled customers.

**Upon entry into the premises for purposes of following-up on the complaint, staff did not ask for a vaccination card and second piece of ID of the EHO.

- Discussed the results of the inspection report with the staff, no signature obtained due to the pandemic.
