Observation: Observed small bait box in storage closet labelled with "POISON" by Orkin.
Corrective Action(s): Poison and bait are NOT ALLOWED to be used inside a food premises, they are to be used for the exterior of the building. Only physical treatment (snap traps, glue boards, tin cats) are permitted inside a food premises.

305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
Observation: Storage closet is cluttered. Bags of laundry are kept on the floor in the middle of the room.
Corrective Action(s): Declutter and reorganize the storage closet to avoid creating harbourage spots for rodents. Keep stored items on the shelving units.

Observation: Cleaning is required in, but not limited to the following areas:
> Storage room: droppings under the sink around the bait box, and few droppings found on the floor among the clutter.
> Behind the grease trap: droppings in the far right corner under the pipes.
> Mop sink: drain flies on the walls.
> Under the 2 compartment sink: droppings behind the chemcial containers.
> Beside the ice machine: droppings and food debris
> Sandwich 2-door and 1-door coolers: food debris inside the cooler on the bottom row.
Corrective Action(s): Remove all droppings and food debris. Clean and sanitize the premises as per your sanitation plan.
> Consistent cleaning and removal of droppings will help to monitor for activity.
> Ensure all areas of the premises are accessible for pest control service.
> Forward the most recent pest control report from Orkin to EHO for review within 1 week.

Observation: 1) High temperature dishwasher is leaking.
2) Display cooler not able to maintain air temperature of 4C or colder at all times.
Corrective Action(s): 1) Repair the leak immediately. Facility is to use single-use utensils and plates as much as possible until the leak is repaired. In-use utensils and plates must be manually washed and sanitized using the 2 comartment sink method - reviewed with staff on site.
