Observation: Food debris collecting on can opener, fry cutter, and frame of utensil drawer.

Mouse droppings in middle of cabinet of large buffet section, in corners around toilets of men's / ladies' washrooms, around door of dividing wall between seating areas, and in kitchen - (on floor behind ovens / fryers, along wall and lower shelving across from fryers, and under soup kettles)
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.

Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine 1 tsp bleach / 1 liter water) after cleaning.

Correction: Product discarded.

To prevent microbial growth potentially hazardous foods such as meat products, cut vegetables, eggs and high moisture grains (soft noodles, soft rice) must be stored at 4 C or colder, unless needed for immediate preparation.

Observation: Food preparation bowls stored on shelves face up with standing water pooling inside and visible food debris.
Correction: Cleaned, sanitized food equipment and utensils are to be stored in a sanitary location that protects them from contamination prior to their next use.

Observation: 10 soiled rags throughout kitchen on counters and food carts without any food contact sanitizer available.

Staff provided peroxide based disinfectant to sanitize food equipment and food contact surfaces. Disinfectant product as per label is only suitable for non-food contact surfaces.

Reservoir of Quaternary Ammonia sanitizer disconnected from feeder system. Premise does not have test strips to verify sanitizer concentration to prepare manually.
Correction: All wiping cloth rags in use are to be stored in a container of sanitizer solution (200 ppm Quaternary Ammonia, to be verified by test strip).

Peroxide disinfectant is not to be used on any food contact surfaces, food utensils and food equipment.

Provide test strips to verify sanitizer concentration.

Observation: Hand soap dispenser at dishwashing station is not dispensing soap.
Correction: Repair soap dispenser to restore proper function.
Comments

Food product stored within cooler and freezer storage units. Ensure food is not removed from cold storage until needed for preparation.

Ensure food portions displayed at buffet sections are discarded after meal period and not returned to the kitchen for later use.

Small hot buffet section: water reservoirs temp 71 - 81 C. Ensure internal temperature is checked for display hot foods throughout meal periods to verify food is maintained at 60 C

Food preparation bowls stored in a sanitary condition.

Quaternary Ammonia sanitizer dispenser installed, dispensing 200 ppm Quaternary Ammonia.

All wiping rags in use are to be stored in 200 ppm Quaternary Ammonia.

Food items observed stored in covered containers.

Food items observed stored off the floor.

Food items no longer stored in opened metal cans.

Pest control technician has provided additional service and has noted completion of outstanding action items.

Premise sanitation has been improved. Laundry room, janitorial room, staff storage area, cabinets below beverage dispensers / ice cream, kitchen ceiling / walls, and dry storage / preparation area cleaning has been conducted.

Dishwasher soap dispenser replaced with temporary dispenser until new wall mount dispenser provided.

Premise closure order complied with and now rescinded. Premise may re-open when ready.

Note:

- Provide plastic bins with fitted lids to store produce deliveries in receiving corridor.
- Provide supply of sanitizer test strips for Quaternary Ammonia
