304 - Premises not free of pests [s. 26(a)] Observation: Significant fruit fly and/or drain fly infestation observed in two key areas: - the back mop sink, beverage syrup, storage room; - the area around the dish sinks. Fly tape in mop room full. Corrective Action(s): A) Continue to target hard-to-reach areas below and around dish sinks and clean thoroughly. Implement cleaning methodology and schedule for all floor drains. B) Investigate condenser lines running out of freezer units in sink area; Ensure they properly drain and there is NO water dripping into cavities where fruit fly breeding habitat can be created. C) Back storage room requires a complete cleaning and organization. Structural deficiencies in this room will be discussed elsewhere in report. Violation Score: 15 311 - Premises not maintained as per approved plans [s. 6(1)(b)] Observation: A) Hot water tank drip tray observed deteriorated and not functional. Drip tray lacks proper drain tie-in. B) Floor in mop sink room deteriorated and impossible to properly clean. This condition is contributing to pest infestation. C) Wall and wall tile behind equipment in kitchen deteriorated and is inihibiting proper cleaning and may also be contributing to pest infestation. D) proper wastewater drainage from some pieces of equipment is missing. Corrective Action(s): A comprehensive renovation plan to correct and fix the above is required. Submit plan with completion timelines to this office within 30 days for review and approval. Violation Score: 5
