HAZARD RATING FOR YOUR FACILITY: Moderate (16-29) Total Score = 28
Critical Hazards: Total Number: 2
205 - Cold potentially hazardous food stored/displayed above 4 °C. [s. 14(2)]
Observation (CORRECTED DURING INSPECTION): Small quanitity of cooked noodles and mashed potatoes (approx two medium bowls) stored in lower compartment of large sandwich cooler. Internal food temperature measured 7 degrees C. Food stored overnight.
Corrective Action(s): Staff voluntarily discarded above-noted food. Ensure cold potentially hazardous food is maintained at 4 degrees C or less to limit the growth of microorganisms (including potential pathogens and their toxins).
Violation Score: 5

302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation (CORRECTED DURING INSPECTION): Food contact surface sanitizer was not available for use at start of inspection.
Corrective Action(s): Staff prepared a bleach water sanitizer at 100 ppm chlorine. Wiping cloth added to sanitizer. Ensure a food contact surface sanitizer is always available for use during operations and ensure it is maintained at the correct concentration for safe and effective sanitizing (e.g. 100 ppm chlorine).
Violation Score: 5

Non-Critical Hazards: Total Number: 4
209 - Food not protected from contamination [s. 12(a)]
Observation (CORRECTED DURING INSPECTION): 1. Open can of puree stored in sandwich cooler.
2. Scoop handle observed in cooked noodles. In addition, scoop observed to be fully submerged in chutney.
3. Potatoes stored under three compartment sink in close proximity to cleaning chemicals.
Corrective Action(s): 1. Ensure that once cans are opened, the contents are immediately transferred to another foodgrade container with a cover if they are to be stored. The interior of the can may potentially degrade over time once cans are opened.
2. Ensure scoops are stored in a manner that protects food from potential contamination. Scoops removed at time of inspection.
3. Ensure food and chemicals are stored separately to protect food from potential contamination (e.g. in the event of chemical leakage). Potatoes moved to another location away from three-compartment sink and chemicals.
Violation Score: 3

304 - Premises not free of pests [s. 26(a)]
Observation: Mouse droppings observed on lower shelf of table used for dry food storage, and on the floor in this area. In addition, droppings observed on the floor to the right of the service counter, and on the lower surface of a small table located in the front area (no food storage). Food not affected (pest proof containers with lids in use).
Corrective Action(s): Ensure all of the above-noted areas are properly cleaned and disinfected. Contact pest control to have the premises serviced within the next week. Continue to ensure food is stored in a manner that protects from contamination by pests. Date to be corrected by: Today.
Violation Score: 3

305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
Observation: 1. Small gap observed under back door, particularly in the middle. In addition, gaps/holes in dry wall observed above the electrical panel.
2. Pooling liquid observed on floor to the left of the large sandwich cooler.
Corrective Action(s): Ensure all gaps that may potentially allow for pest entry are properly sealed. Date to be corrected by: July 19, 2023.
Ensure pooling liquid is removed as this is a potential pest attractant. Date to be corrected by: Today.
Violation Score: 3

308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
Observation: 1. Large sandwich cooler unable to maintain food at 4 degrees C in lower compartment (food at 7 degrees C, ambient temperature at 9 to 10 degrees C). Note: Upper compartment at 4 degrees C.
2. Small leak observed under three-compartment sink.
Corrective Action(s): 1. Ensure above-noted cooler is capable of maintaining food at 4 degrees C or less in both the lower and upper compartments. Have cooler adjusted or serviced as needed. Do not use lower compartment for potentially hazardous food storage until such time. Date to be corrected by: July 19, 2023.
2. Ensure plumbing is repaired to stop leak. Note: Container is available under sink to catch drips. Date to be corrected by: July 19, 2023.
Violation Score: 9
