Observation (CORRECTED DURING INSPECTION): 1. Watched dishwasher pass dirty dishes back and forth between clean and dirty side on the 2 compartment during washing process
2. Found wiping rags left out on counter. .
3. Found spray bottle to have > 200 ppm chlorine
Corrective Action(s): 1. Must ensure dishes go from dirty to clean with out passing through a dirty stage.
2. Ensure wiping rags are kept in the sanitizer bucket between uses.
3. Ensure sanitizer is at least 100 ppm chlorine and not to exceed 200 ppm

Observation (CORRECTED DURING INSPECTION): Front hand sink did not have paper towel at it.
Corrective Action(s): Must ensure all hand sinks are supplied with liquid soap, and paper towel at all times.

Observation (CORRECTED DURING INSPECTION): Observed staff reusing gloves after being removed
Corrective Action(s): Remind all staff of proper hand washing techniques such as when to wash hands (i.e. before putting glove on) and ensure staff are aware they need to change gloves regularly.

Observation: 1. Found bulk items in dry storage that were open no cover in side.
2. Found onions on the floor.
3. Found multiple items put away without covers
Corrective Action(s): 1. All opened bulk items must be placed into a pest proof containers, especially important since mice noted in premises
2. All food must be store 6" off the floor at all times
3. Ensure all items are covered with plastic wrap or lids when not in use.

Observation: 1. Found mice droppings around dishwasher (on top and below).
2. Found bait on top of dishwasher that had been partially eaten.
3. Fruit flies noted in front sushi bar area.
Corrective Action(s): 1. Wash and sanitize all areas where mice droppings noted.
2. Remove all bait from premises immediately; wash and sanitize all areas affected.
3. Have pest control company assist with reducing/eliminating fruit flies from premises.

Pest control is on contract.

Observation: Found back screen door to have gaps around bottom which can allow mice to enter from the outside. Also lots of food debris and water allowing for a food and water source
Corrective Action(s): Must ensure screen door is able to full close each time, and eliminate food and water source.

Observation: Noted heavy grease and food build up through out premises, including but limited to the following areas: Ventilation over deep fryers, under all cooking area, under and behind prep sink by deep fryer, floors through out premises especially in hard to reach areas and ceiling panels such along prep counter.
Corrective Action(s): Have premises thoroughly cleaned, ensuring to get to above noted areas
