Observation (CORRECTED DURING INSPECTION): - Rice scoop kept in water at room temperature with rice particles in water
Corrective Action(s): - Keep in use rice scoop in ice water to minimize microbial growth and to ensure scoop is maintained in sanitary condition. Clean and sanitize scoop at least every 4 hours.

Observation (CORRECTED DURING INSPECTION): 1) Raw beef tendon was noted to be stored above cooked products in cooler.
2) Personal items (lighter, container of mouthwash) present on preparation table
Corrective Action(s): 1) Ensure raw meat products are stored beneath ready to eat items or produce.
2) Do not store personal items in food preparation/service areas.

Observation: - Mouse droppings present along edge of floor in closet behind service area. Droppings appeared to be limited to this area.
Corrective Action(s): - Clean above noted area to remove droppings and disinfect area with bleach solution. Continue to monitor for any further activity ensuring any additional droppings present are cleaned up promptly. Continue to maintain regular pest control service. Have pest control technician place trap in this area.

Comments

- Handsinks (kitchen, server area) were supplied with hot and cold running water, liquid soap and paper towel
- Coolers (in use) were operating at required temperature (4C or colder) and freezers were maintaining food products in a frozen state.
- Temperatures checked:
- 2 door reach in cooler 4C
- 1 door reach in cooler 2C
- 1 door prep cooler 3C (top) 4C (bottom)
- Chest freezers -13C, -19C, -14C
- Hot held foods were maintained above 60C. Cooked rice 66C, Beef broth 70C
- High temperature dishwasher was operational. Final rinse at dish surface measured 72.5C. Operator monitors wash and rinse temperature on external gauges.
- In use wiping cloths kept in bleach sanitizer solution in kitchen and service area. Concentration of solution measured 200ppm.
- General sanitation was satisfactory.
- Floor under cook line well maintained with no accumulation of grease/grime.
- Interior of ice machine appears clean and maintained.
- Pest control contract in place with monthly service.
